Work Exemption Request for COVID-19 Patient Care Form
Employees may request an exemption from work activities surrounding COVID-19 positive or suspected COVID-19 patients awaiting results of testing cases of COVID-19 for medical or religious/deeply held spiritual belief reasons. To request an exemption from work activities surrounding COVID-19 positive or suspected COVID-19 patients awaiting results of testing cases of COVID-19 employees now need to go through the reasonable accommodation request process. Below is a link to the reasonable accommodation request website, which includes additional information related to submitting reasonable accommodation requests and the contact information for Employee Relations. Additionally, the reasonable accommodation request form(s) that need to be completed and submitted to Employee Relations can be found on the website or by contacting Employee Relations directly.

For medical immunization exemption requests a Form C will need to be completed.
Medical exemptions from the vaccination requirement will be allowed only for individuals with a contraindication – or a recognized condition that serves as a reason to not receive the vaccine. This applies to individuals with a documented history of anaphylaxis (life-threatening allergic reaction) or an immediate allergic reaction after a single dose of any of the two-dose vaccines, as well as individuals who developed a rare, severe adverse reaction that has been medically documented by a healthcare provider. In addition, time-limited medical exemptions will be considered for individuals who are currently pregnant, who received a COVID-19 monoclonal antibody treatment for acute COVID-19 illness and other conditions on a case-by-case basis.
